Students need to be approved by the Student Success Center Staff, before you can request any of the following services.
Use this form to request accommodations, if you haven't already been approved.
If you have been approved to receive accommodations through the Student Success Center, you are ready to request the following services electronically.
This form must be completed each time you request test accommodations.
Your request must be made no later than five (5) school days prior to the scheduled test date or
accommodations may be denied.
This form is completed each semester.
The earlier you request the alternative text the earlier your need can be filled.
You can make your request before you purchase the book.
Before you receive your books on alternative text, proof of purchase (copy of receipt) is required.
