Update on Diastolic Function Assessment

In 2025, the American Society of Echocardiography (ASE) published an updated, more contemporary framework that introduces a new stepwise algorithm for estimating left atrial (LA) pressure and grading LV diastolic dysfunction, along with condition-specific pathways for scenarios where the “standard” approach frequently breaks down (e.g., atrial fibrillation, pulmonary hypertension, transplant populations).

This session is designed for practicing sonographers who want practical, protocol-ready confidence with the updated guideline approach. We will walk through how to acquire, optimize, and interpret the core Doppler and 2D parameters used in diastology, then integrate them into an evidence-based conclusion that is consistent, reproducible, and reportable.

Learning objectives

  • Review the updated ASE guidelines for evaluating left ventricular (LV) diastolic function and estimating left atrial (LA) pressure.
  • Explain the pathophysiology of diastolic dysfunction, including the progression from normal to Grades I, II, and III.
  • Identify required echocardiographic parameters for assessing diastolic function, including Mitral Inflow (E/A, DT), Tissue Doppler (e'), LA Volume Index (LAVI), and TR velocity.
