Click on the form that you need. Print the form, then complete, sign and date the form. Make sure your name and NTC Student ID number is on each page of each form. If you are required to also provide support documentation, make sure you submit copies.

| Citizenship Verification | Based upon the information you provided on the Free Application for Federal Student Aid (FAFSA), the Social Security Administration was unable to confirm your US citizenship. |
| Consortium Agreement | Please complete if you are receiving your degree from NTC and you want to receive financial aid for classes you are enrolled in at another school. Please Note: classes must be required for your program at NTC to qualify. If you are receiving your degree from another school, please contact their Financial Aid department to fill out a Consortium Agreement with them. |
| Homeless/Unaccompanied Youth Verification | Use this form if you reported on your FAFSA that you were an unaccompanied youth who is homeless or are an unaccompanied youth providing for your own living expenses who is at risk of being homeless. |
| Identity and Statement of Educational Purpose | The U.S. Department of Education has implemented regulations, which requires some applicants to submit a Statement of Educational Purpose and a copy of the front and back of a valid government issued photo identification (ID) such as a driver’s license, passport or other state-issued ID. |
| Parent Marital Status / Tax Filing Status Conflict | Use this form if there is a conflict between what was reported for parent’s current marital status and the filing status of their 2023 income tax return. |
| Parent Refusal to Provide FAFSA Information | If you have a situation where your parent refuses to provide their information on the 2025-2026 FAFSA and refuses to provide you with financial support, please complete and submit this form. |
| Physician’s Certification and Borrower’s Acknowledgement of Obligation | If you have a situation where your parent refuses to provide their information on the 2025-2026 FAFSA or refuses to to provide you with financial support, please complete and submit this form. |
| Professional Judgment Request Due to Special Circumstances | A special circumstance is when the information provided on the FAFSA form is not reflective of the family’s and/or student’s financial situation. A financial aid administrator may then adjust a student’s FAFSA data to more accurately reflect the current situation. Note: There must be a significant change to the household finances to be considered for a Professional Judgment. Circumstances that will be considered:

| Request for Dependency Override Due to Unusual Circumstances | Student dependency status is determined by information provided on the FAFSA and required by the US Department of Education. Wausau Campus (enter through Main Entrance and deliver to Student Services) or drop-off at any of NTC’s regional campus locations, leaving it with the regional campus front desk staff. Incomplete forms will be returned to you and may cause a delay in processing your aid application.

Dynamic Forms
Dynamic Forms is a web-based platform the college uses to manage certain verification forms. Each Dynamic Form is linked to a unique URL, which is included in specific verification action items assigned in Workday.
This electronic form submission process allows students to securely submit requested documentation to the Financial Aid Office. Dynamic Forms also supports electronic signatures, which display the user's name in signature format along with a timestamp that includes the date and time of submission.
